Job summary

Enhabit Home Health & Hospice is seeking qualified clinicians to represent the branch in external transitional care activities and engage with patients, families, physicians, hospitals, and community partners. The role focuses on delivering patient-centered transition plans that improve quality and efficiency while promoting the services Enhabit provides.

The position requires state licensure, graduation from an approved clinical education program, at least one year in home care, knowledge of

Qualifications

• Must be licensed in the state where they currently practice.
• Must be a graduate of an approved school of clinical education.
• Must have at least one year experience in home care.
• Must have demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services. Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible for education related to provided services.
