Job summary

Enhabit Home Health & Hospice is seeking qualified professionals to represent the branch in external transitional care activities and engage with patients, families, physicians, and hospitals to promote Enhabit services.

The role involves developing patient-centered transition plans by integrating clinical guidelines and metrics to ensure quality and efficiency, while also serving as a public education resource for the provided services.

Qualifications

• Must have basic demonstrated technology skills, including operation of a mobile device.
• Education and experience, essential: graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws and regulatory guidelines governing hospice operations.
• Preferred: demonstrated hospice experience.

Responsibilities

• Represent the branch in external transitional care activities with patients, families, physicians, hospitals, and health groups to apprise them of Enhabit services.
• Develop patient-centered transition plans by integrating clinical guidelines, protocols, and metrics to promote quality and efficiency.
• Serve as a public awareness representative and education resource for provided services.
