Job summary

The role requires licensure, a clinical education background, and a year of home-care experience, plus strong knowledge of applicable laws and the use of mobile devices to coordinate care.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated understanding of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, and other health groups to apprise them of Enhabit services.
• Integrate clinical guidelines, protocols and metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and educate about provided services.
