Job summary

Enhabit Home Health & Hospice is seeking a qualified professional to represent the branch in external transitional care activities and to engage patients, families, and partners.

The role focuses on integrating clinical guidelines and education to develop patient-centered transition plans that promote quality and efficiency.

Candidates must be licensed in their state, a graduate of an approved school of clinical education, and have at least one year of home care experience.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, senior living communities and related health groups to inform them of Enhabit services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and be responsible for education related to provided services.
