Job summary

Enhabit Home Health & Hospice is seeking a clinically licensed professional to support patient transitions and community engagement from our Lawrenceville, GA branch. The role focuses on educating caregivers and partners about Enhabit services and coordinating compliant, high-quality care plans.

The ideal candidate will bring at least one year in home health, possess state licensure, and demonstrate knowledge of applicable hospice regulations along with basic mobile tech skills.

Qualifications

• Must be licensed in the state where they practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws governing hospice operations.
• Must have basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to apprise them of Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans that integrate clinical guidelines to promote quality and efficiency.
• Serve as a public awareness representative and educate about provided services.

Education

Graduate of an approved school of clinical education

Licensed in the state where they practice
