Job summary

Enhabit Home Health & Hospice is seeking a healthcare liaison to represent the branch in external transitional care activities and professional contacts with patients and partners. You will integrate clinical guidelines to develop patient-centered transition plans that promote quality and efficiency.

Additionally, you will serve as a public awareness representative and educate stakeholders about the services provided, ensuring compliance with applicable laws and regulations.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where practicing.
• At least 1 year experience in home care.
• Knowledge of federal, state, and local laws governing hospice operations.
• Basic technology skills including use of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and in professional contacts to promote Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans by integrating clinical guidelines and protocols.
• Educate patients, families, and community partners about provided services.
