Job summary

Compassus is seeking a Hospice Team Coordinator in Reston, VA to oversee the delivery of hospice care, coordinate patient data, and support the interdisciplinary team. You will maintain accurate records, ensure smooth communication, and promote compliance across departments while prioritizing patient comfort and dignity.

Qualifications

• High school diploma or GED required.
• At least two years of experience with medical records, medical office, or business office support preferred.

Responsibilities

• Accurately enters and manages patient data including referral intake information and updates to patient demographics.
• Ensures admission documentation is complete with physician/medical director's CTI and order to admit; uploads to HCHB.
• Maintains patient records in HCHB, generates reports for regulatory compliance, and adds coordination notes as needed.
• Monitors DME and medical supplies orders and informs management of trends or issues.
• Verifies and uploads paper-based visit records into the EMR and HCHB for complete patient histories.
• Ensures organized document storage in patient charts for easy retrieval.
• Verifies care levels match billable care and supports accurate billing/reimbursement.
• Manages decertification paperwork and notifies providers of patient discharge.
• Coordinates with the IDT for holistic patient care; serves as central contact for patients and families.
• Handles calls, takes messages, and transfers to appropriate staff; manages phone systems.
• Assists the Hospice Director of Clinical Services with pre- and post-IDT activities, including agendas and UPOCs, and sends UPOCs to physicians.
• Prepares binders for patients in facilities and performs other duties as assigned.
