Job summary

Compassus is seeking a Hospice Team Coordinator to manage patient data, admissions documentation, and care coordination across the interdisciplinary team. The role emphasizes accurate record-keeping in HCHB and EMR systems, timely processing of orders, and effective communication with families and clinicians.

The coordinator will ensure documentation aligns with billing requirements, support IDT activities, and assist with pre- and post-IDT tasks to promote high-quality hospice care.

Qualifications

• High school diploma or GED required.
• At least two years of experience with medical records, medical office, or business office support preferred.
• Strong written and verbal communication skills.

Responsibilities

• Accurately enters and manages patient data and referrals.
• Ensures admission documents are complete with physician/medical director CTI.
• Uploads documents into HCHB and maintains patient records in EMR.
• Monitors DME orders and coordinates with management on trends.
• Verifies billing levels of care and supports med record retrieval for care coordination.
