Job summary

Affinity Health Management is seeking a Hospice Aide to assist patients under RN supervision. You will support personal care, comfort, and safety, while documenting observations per the plan of care.

The role emphasizes compassionate end-of-life support, adherence to care standards, and effective communication with patients and families. Applicants should be CNA-certified and 18+, with a valid driver’s license and CPR/medical training.

Qualifications

• Completion of a state approved certified nursing assistant course.
• Current Healthcare Provider CPR certification.
• COVID-19 vaccination as required by facilities.
• High school diploma or GED or equivalent.
• Ability to read and follow written instructions and document care.
• Understanding hospice philosophy and specialized care for terminally ill.
• Prior hospice, orderlie, acute, rehabilitative, or long-term care experience preferred.
• Basic computer skills to document in EMR.
• 18 years of age or older.
• Valid driver's license and insured vehicle per organization policy.

Responsibilities

• Provide personal care including bathing, grooming and linen changes.
• Assist with feeding and assistance with daily activities.
• Poor conditions and safety checks, using equipment properly.
• Report pain, changes, or concerns to RN Case Manager.
• Assist in ambulation, transfers, and range of motion.
• Maintain clean and orderly patient living area.
• Adhere to care procedures and documentation standards.
