Affinity Health Management is seeking a Hospice Chaplain to provide spiritual care to patients and families, coordinating with the care team to address spiritual needs. The role involves direct care, education, and liaison work with community chaplains and volunteers.

Qualifications

• Demonstrates strong communication skills, both verbal and written.
• Ability to organize and manage multiple care plans.
• Cultural and faith sensitivity in diverse settings.
• CPR certification and willingness to maintain credentialing.
• Experience with Clinical Pastoral Education (CPE) preferred.

Responsibilities

• Assess patients’ and families’ spiritual needs and plan care accordingly.
• Provide direct spiritual care to patients and families/caregivers.
• Coordinate with community chaplains and spiritual counselors.
• Maintain records of spiritual care services for QA and policy review.
• Attend IDG/meetings and advise on spiritual care within the team.
• Recruit community chaplains and volunteers to meet needs.
