Compassus is seeking a Hospice Social Worker in Missouri to provide psychosocial assessments, counseling, and care coordination. You will collaborate with the interdisciplinary team to develop and implement plans of care, address family and patient needs, and connect families with community resources.

Qualifications

• Explains hospice services and Medicare benefits; obtains consent and election of benefits.
• Submits documentation to facilities; ensures timely completion per policy.
• Participates in IDT; assists in plan of care development and implementation.
• Provides psycho-social support and counseling to patients and families; leverages external resources as needed.

Responsibilities

• Performs psychosocial assessments; addresses financial, housing, caregiver stressors, and community resources.
• Facilitates placement in long-term care as warranted and monitors adjustment.
• Attends care planning meetings at long-term care facilities.
• Supports advance care planning including living wills and POA documents.
