Job summary

Compassus is seeking a Hospice Social Worker in Missouri to support patients and families with psychosocial care. The role collaborates with the IDT to develop and implement care plans, coordinate resources, and promote advance planning, while upholding Compassus values and ethical standards.

The ideal candidate holds an MSW (CSWE-accredited) with healthcare experience, or a BS in social work with supervision for MSW, and must meet state licensure requirements as applicable.

Qualifications

• MSW from CSWE-accredited program with at least 1 year healthcare experience.
• BSW with 1 year healthcare experience will be considered if MSW is available for supervision.
• Missouri: no licensure required; BS must be supervised by MSW where applicable.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent.
• Submit documentation to facilities after patient visits per policy.
• Participate as a member of the IDT and assist in the plan of care.
• Provide psychosocial support and counseling; coordinate with external resources as needed.
• Maintain up-to-date care plans reflecting problems, goals and interventions.
• Facilitate advance planning including living will/POA, funeral arrangements, and memorial services.
• Support care planning meetings at long-term care facilities.
