Job summary

Compassus is seeking a Hospice Social Worker in Missouri West Plains to provide psychosocial support, counseling, and resource coordination within the interdisciplinary team. You will assess needs, facilitate advance planning, and support families in coping with serious illness and end-of-life issues.

The role emphasizes collaboration, comprehensive care planning, and sensitivity to diverse backgrounds, with a focus on maintaining patient and family well-being and quality of life.

Qualifications

• MSW from CSWE-accredited program with at least 1 year of healthcare experience.
• Bachelor’s degree in social work or related field with 1 year healthcare experience may be considered for locations with MSW supervision.
• State licensure as required and applicable for the state of practice.

Responsibilities

• Explain hospice services and Medicare benefits; obtain informed consent and election documents.
• Submit documentation to facilities post visits according to policy.
• Participate as IDT member and contribute to plan of care.
