Access Hospice Care, part of the Optum/LHC Group family, is hiring a Social Worker in Springfield, Missouri. You will provide medical social services under physician guidance to help patients and families understand social and emotional factors affecting health, and contribute to our interdisciplinary care team.

Primary responsibilities include supporting patients, families, and clinicians in care planning and connecting them with community resources.

Qualifications

• Bachelor of Social Work OR Masters of Social Work.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Assists patients and care teams to understand social factors related to health.
• Contributes to development of a comprehensive Plan of Care.
• Instructs team on community resources for patient support.
