Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice.
• Education: MS degree in social work from an accredited school of social work approved by the CSWE.

Responsibilities

• Perform psychosocial evaluations and ongoing counseling for patients and families.
• Provide emotional support during end-of-life journey.
• Work in accordance with the plan of care and collaborate with leadership.
