Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs and coordinate hospice-related support. The role collaborates with interdisciplinary teams and may involve variable hours based on census and program needs.

The ideal candidate has a Master’s in Social Work, hospice experience of 1+ year, and licenses as LSW or LCSW with IDFPR, plus a valid driver’s license and auto insurance.

Qualifications

• Master’s Degree in Social Work.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) with IDFPR.
• Driver’s License and auto insurance as required.
• Hospice experience of 1+ years.
• Ability to work as part of a team.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs related to diagnosis and care.
• Develop a bereavement plan of care addressing family member needs.
• Interpret and communicate patient/family faith and culture traditions and consider their impact on care.
• Educate patients and families, overcoming barriers and matching learning needs.
• Document assessments and MSW interventions in the electronic medical record.
• Implement and adjust social work plans to maximize benefits and coping.
