Job summary

Carle Health is seeking an experienced Home Services Social Worker to assess psychosocial needs of patients and families, collaborate with interdisciplinary teams, and provide crisis intervention when needed. The role requires hospice experience of 1+ years and offers a sign-on bonus.

Education includes a Master’s Degree in Social Work with licensure as LSW/LCSW in the applicable state. Travel within service area and electronic charting are expected.

Qualifications

• Master's Degree in Social Work or related field.
• Licensure as Social Worker (LSW) and/or LCSW in applicable state.
• Hospice experience of 1+ years preferred.

Responsibilities

• Perform psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement care plans and provide support.
• Educate patients and families, addressing barriers and learning needs.
• Document assessments and care plans in electronic medical records.
