Job summary

Carle Health seeks a Home Services Social Worker to identify psychosocial needs of patients and families within an interdisciplinary team. The role includes resource support, short-term counseling, and crisis intervention, with a Hospice requirement of 1+ years.

The position offers a sign-on bonus and requires a Master’s in Social Work and licensure (LSW or LCSW) compatible with state regulations; a Driver’s License and proof of auto insurance are also required.

Qualifications

• Master’s Degree in Social Work.
• Licensure: LSW or LCSW (IDFPR).
• Driver’s License required.
• Proof of Auto Insurance required.
• Hospice experience of 1+ years.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
• Develop a bereavement plan of care addressing family/caregiver needs.
• Interpret and communicate patient/family faith and culture; consider impact on care.
• Educate patients and families to overcome barriers and match learning capabilities.
• Complete and transmit visit notes and orders timely; update care plans as needed.
• Document psychosocial assessments, financial assessments, and MSW interventions in EMR.
• Implement social work plan to enhance family strength, utilization of resources, benefits, and coping skills.
• Support patient decisions and life care planning; collaborate with IDT.
• Maintain care per plan; reflect palliative philosophy in daily practice.
