Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families, coordinating care with interdisciplinary teams. Qualifications

• Master's degree in Social Work; licensure in IL (LSW/LCSW) required.
• Hospice experience of 1+ year is required.
• Ability to work as part of a team.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develops a bereavement plan of care to address family member/caregiver needs.
• Interprets and communicates pt/family faith and culture traditions; acknowledges spiritual beliefs affecting care.
• Educates patient and family to overcome barriers; matches learning capabilities and needs.
• Documents psychosocial assessments and interventions in the EMR; collaborates with IDT on care plans.
