Qualifications

• Master's Degree in Social Work
• Social Work field of study required
• Hospice experience 1+ years preferred
• Licensed Social Worker (LSW) / Licensed Clinical Social Worker (LCSW) in IL or applicable state

Responsibilities

• Perform psychosocial assessments of patients and families.
• Develop bereavement plans of care.
• Address spiritual and cultural considerations in care planning.
• Educate patients and families to overcome barriers and meet needs.
• Document assessments and interventions in the EMR.
• Collaborate with the IDT to implement social work plans.
