Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs of patients and families, coordinating with interdisciplinary teams. The role involves assessments, planning, and crisis intervention within a hospice context.

Requires a Master’s in Social Work and licensure (LSW or LCSW) with 1+ year hospice experience. A driver’s license is required and a sign-on bonus is offered.

Qualifications

• Master's Degree in Social Work (MSW).
• Licensure as Social Worker (LSW) or Clinical Social Worker (LCSW) per IDFPR.
• Hospice experience required (1+ year).
• Driver's license and automotive insurance as required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plans of care.
• Educate patients and families about learning needs and cultural beliefs.
• Document assessments and interventions in EMR.
• Collaborate with IDT to implement social work plan.
• Identify and refer transportation, food, and utility assistance.
