Qualifications

• Master’s degree in Social Work.
• Licensure as LSW or LCSW per IDFPR.
• Hospice experience of 1+ years.
• Driver’s License and proof of Auto Insurance.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care.
• Educate patients and families respecting faith/cultural beliefs.
• Document assessments and MSW interventions in EMR.
• Complete or assist with social work plan of care in collaboration with IDT.
