Qualifications

• Master's degree in social work is required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) licensure per IDFPR.
• Driver's license and proof of auto insurance.
• Hospice experience of 1+ year is required.
• Ability to work as part of a multidisciplinary team.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation.
• Develop a bereavement plan of care addressing family member and caregiver needs.
• Educate patients and families, overcoming barriers and matching learning levels.
• Document psychosocial assessments, financial assessments, and MSW interventions in the EMR.
• Implement social work plans to optimize family strength, resources, medical benefits, and dignity at end of life.
