Job summary

Commonwealth Hospice, part of the LHC Group and Optum family, seeks a Social Worker - Hospice Per Diem to provide medical social services under physician direction within an interdisciplinary team. The role focuses on understanding social and emotional factors impacting health and helping patients cope.

This PRN in-home position requires a Master's in Social Work, a Virginia social work license, and at least 1 year of healthcare social work experience, with travel within a 50-mile radius.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work.
• Virginia social work license required.
• Home-based hospice experience preferred.

Responsibilities

• Assists patients and families to understand personal, emotional and social factors affecting health.
• Contributes to a comprehensive Plan of Care with the care team.
• Instructs team members on community resources and bereavement needs.
• Function as Bereavement Coordinator and oversee bereavement services when needed.
