Job summary

Elite Hospice, part of the LHC Group and UnitedHealth Group family, is seeking a Social Worker - Per Diem to provide medical social services under physician direction and an interdisciplinary team. This role focuses on understanding social and emotional factors related to the patient''s health status and developing coping mechanisms.

Responsibilities include assisting patients and families, planning a daily Plan of Care, guiding the care team on community resources, and serving as Bereavement

Qualifications

• Requires Bachelor or Master of Social Work.
• Current driver''s license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Assist patients, families, physician and care team to address social and emotional factors affecting health.
• Contribute to the development of an integrated Plan of Care on a daily basis.
• Instruct care team on community resources available to assist patients as needed.
• Serve as Bereavement Coordinator and plan bereavement services when needed.
