Job summary

Elite Hospice, part of the LHC Group, is seeking a Social Worker – Hospice Per Diem (LMSW) to assist patients and families. You will participate in developing and implementing a comprehensive Plan of Care in collaboration with physicians and the Interdisciplinary Group, addressing emotional and social factors impacting health.

Per diem hours and regional pay apply. Qualifications include a Master's in Social Work, 1+ year health care social work experience, and a valid driver's license.

Qualifications

• Master's Degree in Social Work from a school accredited by the Council on Social Work
• 1+ years of social work experience in health care setting
• Valid driver's license, vehicle insurance, access to reliable transport

Responsibilities

• Assist patients and families to understand social factors related to health status
• Contribute to an integrated Plan of Care with the health care team
• Instruct care team on resources available to patients
• Possible Bereavement Coordinator responsibilities and plan of care for bereavement needs
