Job summary

CHRISTUS VNA Hospice, a part of LHC Group and UnitedHealth Group family, is seeking a Social Worker - Hospice Per Diem to provide medical social services under physician and IDG oversight, addressing social and emotional factors affecting patient health.

Primary responsibilities include assessing patients and families, contributing to the Plan of Care, coordinating community resources, and bereavement services when needed.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• 1+ years of social work experience in a health care setting
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Licensed Social Worker in the state of practice (preferred)
• CPR certification or ability to complete within 90 days of hire (preferred)
• Bereavement Coordination experience (preferred)
• Experience establishing a plan of care for bereavement needs (preferred)

Responsibilities

• Assists patient and family to understand significant social factors related to health status
• Contributes to development of an integrated Plan of Care for patients daily
• Instructs staff on community resources to assist patients as needed
• Able to function as Bereavement Coordinator and supervise bereavement services
