Job summary

UnitedHealth Group's CHRISTUS VNA Hospice, part of LHC Group, is seeking a Social Worker - Hospice Per Diem to provide medical social services under physician direction and Interdisciplinary Group guidance. The role focuses on understanding social and emotional factors affecting patient health and developing coping strategies.

Primary responsibilities include assessing patients and families, contributing to Plan of Care, linking to community resources, and Bereavement coordination when needed.

Qualifications

• MSW from an accredited school of Social Work.
• 1+ year of social work experience in a health care setting.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Assists patient and family to understand personal, emotional, environmental, and social factors related to health status.
• Contributes to development of a comprehensive Plan of Care on a daily basis.
• Instructs care team on community resources available to assist patients.
• Bereavement coordination when needed and planning bereavement services.
