Candidates must have a Master’s in Social Work and Virginia licensure, with 1+ year of health-care social work experience and access to reliable transportation.

The role requires travel within a 50-mile radius and primarily operates during standard business hours, with preference for Home-Based Hospice experience.

Qualificações

• Master's Degree from a school of Social Work accredited by the Council of Social Work.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsabilidades

• Assists patients and care teams to understand social factors affecting health.
• Develops a comprehensive Plan of Care with the health care team.
• Instructs staff on community resources to support patients, as needed.
• Can serve as Bereavement Coordinator and address bereavement needs with a clear plan.
