The role involves supporting patients and families, planning daily care, guiding the team on community resources, and coordinating bereavement services when needed. Requires a BS or MSW and a valid driver’s license.

Qualifications

• Bachelor of Social Work OR Master of Social Work.
• Current Driver's License and access to reliable transportation.

Responsibilities

• Understand significant social, emotional, environmental factors related to health status.
• Contribute as a health care team member to develop a comprehensive Plan of Care.
• Plan of Care for patients on a daily basis.
• Instruct health care team on community resources for patients.
• Bereavement coordination and planning of bereavement services as needed.
