Job summary

St. Croix Hospice is seeking a dedicated Social Worker to join the patient care team and provide compassionate emotional and practical support for patients and families within their Plan of Care.

The role involves meeting with patients and loved ones, collaborating with clinicians, and helping families access community resources and services. A CSWE-accredited degree and state licensure are required.

Qualifications

• Bachelor’s degree in social work from a CSWE-accredited program.
• Current Social Work license in the state(s) of practice.
• Ability to pass background, consumer report, and drug test.
• Reliable transportation and valid driver’s license with auto insurance.

Responsibilities

• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
• Provide counseling, crisis intervention, and bereavement support, including follow up for at‑risk individuals and coordination of bereavement activities.
• Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end-of-life needs, liaise with community agencies, and ensure HIPAA compliance.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance.
