Job summary

St. Croix Hospice seeks a compassionate Social Worker to join the patient care team, providing emotional and practical support to patients and families within their Plan of Care.

You will assess psychosocial status, coordinate with the clinical team, and connect families to community resources, including bereavement services. Under MSW supervision, you will educate families about advanced directives, maintain records, and participate in group meetings.

Qualifications

• Bachelor's degree in social work is required.
• Current license to practice social work in the state(s) of practice is required (LBSW/LMSW/LICSW).
• Background check, consumer report, and drug test may be required.
• Valid State Driver’s License and access to a vehicle in good working order are required.

Responsibilities

• Assess psychosocial status of patients and families in relation to terminal illness.
• Provide crisis intervention and bereavement support.
• Educate patients and families on advanced directives and community resources.
• Coordinate with the interdisciplinary care team and maintain records.
