Job summary

St. Croix Hospice seeks a compassionate Social Worker to join the patient care team, providing emotional and practical support to patients and families as outlined in the Plan of Care.

You will meet with patients, collaborate with clinicians, and help families access community resources. You will conduct psychosocial assessments, offer counseling and crisis intervention, and coordinate bereavement activities while ensuring thorough documentation and HIPAA compliance.

Qualifications

• Bachelor’s degree in social work from CSWE-accredited program.
• Current Social Work license in state(s) of practice.
• Ability to pass background, consumer report, and drug test.

Responsibilities

• Conduct psychosocial assessments, identify safety or support needs, and provide resources, referrals, and care planning support for patients and families.
• Provide counseling, crisis intervention, and bereavement support, including follow up for at risk individuals and coordination of bereavement activities.
• Collaborate with the interdisciplinary team, maintain accurate documentation, support cultural and end of life needs, liaise with community agencies, and ensure HIPAA compliance.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance.
