Job summary

St. Croix Hospice in Moose Lake, MN is seeking a Social Worker to support patients and families at end-of-life, coordinating with the clinical team and connecting them with community resources.

The role requires assessing psychosocial status, counseling, and assisting with advance directives within the Plan of Care. Under MSW supervision, you will provide bereavement support, coordinate with healthcare providers, and help families navigate challenging decisions, while traveling to patient homes

Qualifications

• Bachelor’s degree in social work from an accredited program.
• Current social work licensure in practice state(s).
• Ability to pass background, consumer report and drug test.
• Valid State Driver’s License and reliable auto for travel.

Responsibilities

• Assess psychosocial status of patients and families and relay findings to the care team.
• Coordinate with clinical team and community resources for plans of care.
• Provide crisis intervention and bereavement support as needed.
• Educate and assist with advance directives and care planning.
• Maintain confidential clinical records and ensure HIPAA compliance.
