Job summary

St. Croix Hospice in Moose Lake, MN is seeking a Social Worker to support patients and families on their end-of-life journey. You will assess psychosocial needs, coordinate with the care team, and connect families with resources.

Qualifications

• Bachelor's degree in social work from CSWE-accredited program.
• Current licensure to practice social work in the state(s) of employment.
• Willingness to travel as needed for home-based care.

Responsibilities

• Assess psychosocial status of patients and families and document findings.
• Coordinate with clinical team and community resources to support plans of care.
• Counsel patients and families, provide crisis intervention when needed.
• Provide ongoing bereavement support and educate about advanced directives.
• Maintain confidential medical records and ensure HIPAA compliance.
