Job summary

St. Croix Hospice seeks a compassionate Social Worker to support patients and families as part of the care team. You will meet with patients, collaborate with clinicians, and help families access community resources and services in alignment with each Plan of Care.

The role emphasizes compassionate support, autonomy in practice, and meaningful connections with patients and families within a full-time schedule.

Qualifications

• Bachelor’s degree in social work from a CSWE-accredited program.
• Current Social Work license in practicing state(s).
• Four licensure levels: LSW, LGSW, LISW, LICSW.
• Ability to pass background, consumer report, and drug test.
• Conduct psychosocial assessments and care planning support for patients and families.
• Provide counseling, crisis intervention, and bereavement support; coordinate resources and referrals.

Responsibilities

• Meet with patients and families to assess needs and provide resources.
• Collaborate with the interdisciplinary clinical team and community agencies.
• Assist with care planning, resources access, and coordination of services.
• Maintain accurate, HIPAA-compliant documentation and follow-up with at-risk individuals.
