St. Croix Hospice is seeking a compassionate Social Worker to join our patient care team. You will meet with patients and families, collaborate with clinicians, and help connect families with community resources as part of each Plan of Care.

The role emphasizes counseling, crisis intervention, and bereavement support, while coordinating with the interdisciplinary team and maintaining HIPAA-compliant documentation.

Qualifications

• Bachelor’s degree in social work from an institution accredited by the CSWE.
• Current Social Work licensed in the state(s) of practice (CSW/CAPSW/CISW/LCSW/SWTC).
• Ability to pass background, consumer report, and drug test.

Responsibilities

• Conduct psychosocial assessments and identify safety or support needs.
• Provide counseling, crisis intervention, and bereavement support.
• Coordinate with interdisciplinary team and ensure HIPAA compliance during documentation.
