Job summary

St. Croix Hospice seeks a compassionate Social Worker to provide emotional and practical support to patients and families within the Plan of Care. You will meet with families, collaborate with the clinical team, connect resources, and assist with care planning, counseling, and bereavement support.

The role emphasizes autonomy, meaningful patient connections, and alignment with HIPAA standards, with a typical M-F, 8–5 full-time schedule and opportunities for professional development.

Qualifications

• Bachelor’s degree in social work from an CSWE-accredited program.
• Current Social Work license in state(s) of practice.
• Licensure levels: LBSW, LSW, LCSW.
• Ability to pass background, consumer report, and drug test.
• Conduct psychosocial assessments and care planning support for patients/families.
• Provide counseling, crisis intervention, and bereavement support with follow-up.
• Collaborate with interdisciplinary team and maintain HIPAA-compliant documentation.
• Reliable transportation, valid driver's license, and auto insurance.

Responsibilities
