This position requires an MSW, state licensure, and 2+ years in a health‑care setting. The role includes evenings/weekends on-call and travel within the service area, with competitive hourly pay and benefits.

Qualifications

• Master’s degree required; CSWE-accredited program.
• State licensure/certification required where applicable.
• Minimum 2+ years in health care or social services.
• Hospice/palliative care experience preferred.
• Comfort with grief, loss, and end-of-life conversations.
• Strong community resource knowledge and care coordination.
• Excellent documentation and compliance focus.
• Self-directed with minimal supervision.
• Valid driver’s license; willingness to travel locally.

Responsibilities

• Conduct psychosocial, spiritual, and pre-bereavement assessments for patients and families.
• Provide ongoing emotional support and counseling throughout care.
• Connect families with community resources and financial assistance.
• Guide families through Advanced Directives with clarity.
• Assist with facility placement when appropriate.
• Maintain accurate clinical documentation at residence and in agency records.
• Participate in interdisciplinary team and IDG meetings to shape care plans.
• On-call duties evenings and weekends as needed.
• Ensure compliance with local, state, and federal laws and Medicare regulations.
