Transitions Hospice in Rockford, IL is seeking a part-time Social Worker to provide psychosocial support to patients and families within a hospice program. You will manage a meaningful caseload and collaborate with an interdisciplinary team to guide care plans and community resources.

Requirements include a Master’s in Social Work, state licensure, 2+ years in healthcare or social services, and a valid driver’s license with willingness to travel locally. Hospice experience is preferred.

• Minimum of 2+ years social service experience in a healthcare setting or social services agency.
• Experience in hospice, palliative care, or a related healthcare setting preferred.
• Strong knowledge of community resources and care coordination.

Responsibilities

• Conduct psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers.
• Provide ongoing emotional support and counseling throughout the care journey.
• Identify and connect families with community resources and financial assistance.
• Guide families through Advanced Directives with clarity and sensitivity.
• Assist with facility placement for patients when appropriate.
• Maintain accurate clinical documentation at the patient's residence and in agency records.
• Actively participate in interdisciplinary team and IDG meetings to shape care plans.
• Participate in on-call duties on evenings and weekends as needed.
• Ensure compliance with local, state, and federal laws and Medicare regulations.

Schedule: Mon- Friday (8 hr. shifts) Part-Time less than 32 hours a week

What You'll Do

As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:

• Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
• Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
• Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
• Guiding families through Advanced Directives with clarity and sensitivity
• Assisting with facility placement for patients when appropriate
• Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
• Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
• Participating in on-call duties on evenings and weekends as neededli>
• Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

What You Bring

• Master’s degree required. Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
• Licensure and/or certification as required by the State.
• Minimum of 2+ years social service experience in a health care setting or social services agency.
• Experience in hospice, palliative care, or a related healthcare setting preferred
• Deep comfort with grief, loss, and end-of-life conversations
• Strong knowledge of community resources and care coordination
• Excellent documentation skills and attention to compliance
• Self-directed with the ability to work with little supervision
