Transitions Hospice in Rockford, IL is seeking a licensed Social Worker for a part-time role. You will carry a meaningful caseload, advocate for patients and families, and coordinate community resources to support end-of-life care.

This position requires an MSW, state licensure, and 2+ years in a health‑care setting. The role includes evenings/weekends on-call and travel within the service area, with competitive hourly pay and benefits.

Qualifications

• Master’s degree required; CSWE-accredited program.
• State licensure/certification required where applicable.
• Minimum 2+ years in health care or social services.
• Hospice/palliative care experience preferred.

Salary: $35.00 - $38.00 / hour

Schedule: Mon- Friday (8 hr. shifts) Part-Time less than 32 hours a week

What You'll Do

As a Transitions Hospice Licensed Social Worker, you'll carry a meaningful caseload and serve as a trusted advocate and counselor for patients and families throughout the hospice journey. Your responsibilities include:

• Conducting thorough, compassionate initial psychosocial, spiritual, and pre-bereavement assessments for patients, families, and caregivers
• Providing ongoing emotional support and counseling as a consistent presence throughout the care journey
• Identifying and connecting families with community resources — including financial assistance — to meet practical and emotional needs
• Guiding families through Advanced Directives with clarity and sensitivity
• Assisting with facility placement for patients when appropriate
• Maintaining accurate, timely clinical documentation at the patient's residence and in agency records
• Actively contributing to interdisciplinary team and IDG meetings to shape holistic care plans
• Participating in on-call duties on evenings and weekends as neededli>
• Managing caseload while Assuring compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures

Skills, Knowledge and Expertise

What You Bring

• Master’s degree required. Master’s degree from school accredited by the Council on Social Work Education required in states as specified.
• Licensure and/or certification as required by the State.
• Minimum of 2+ years social service experience in a health care setting or social services agency.
• Experience in hospice, palliative care, or a related healthcare setting preferred
• Deep comfort with grief, loss, and end-of-life conversations
• Strong knowledge of community resources and care coordination
• Excellent documentation skills and attention to compliance
• Self-directed with the ability to work with little supervision
• Valid driver's license and willingness to travel locally for patient visits
