Confidential Hospice Provider seeks a compassionate Hospice Medical Social Worker (MSW/LSW) to support patients and families in end-of-life care. You will assess psychosocial needs, coordinate with the interdisciplinary team, and provide counseling, bereavement support, and community resource connections.

The role requires an MSW from an accredited program and IL LSW/LCSW licensure, with hospice or related healthcare experience preferred.

Qualifications

Responsibilities

• Facilitate family meetings and coordinate care team communication.

• Maintain accurate clinical documentation per hospice standards.

About the Role:

Local Hospice Company is looking for a compassionate and dedicated Hospice Medical Social Worker (MSW/LSW) to join our caring team. Responsibilities:

• Conduct psychosocial assessments to evaluate patient and family needs, strengths, and coping abilities
• Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team
• Provide counseling and emotional support to patients and families facing terminal illness and grief
• Connect patients and families with community resources, financial assistance programs, and support services
• Assist with advance care planning, including advance directives and end-of-life decision-making
• Facilitate family meetings and coordinate communication between patients, families, and the care team
• Provide bereavement support and follow-up services to families after the loss of a loved one
• Maintain accurate and timely clinical documentation in compliance with hospice regulations and standards
• Participate in total plan of care development, interdisciplinary team (IDG) case conferences, and comprehensive discharge planning as needed.

Requirements:

• Master's degree in Social Work (MSW) from an accredited program required
• Current LSW or LCSW licensure in the state of Illinois
• Prior experience in hospice, palliative care, home health, or a related healthcare setting preferred
• Strong knowledge of end-of-life care, grief counseling, and community resource coordination
• Excellent interpersonal and communication skills with a compassionate, patient-centered approach
• Ability to work independently and collaboratively within an interdisciplinary care team
• Valid driver's license and reliable transportation for patient home visits in the Saint Charles, IL area
