Job summary

UP Health System Hospice, part of the LHC Group, is seeking a part-time Social Worker in Michigan. You will provide medical social services under physician direction, helping patients and families cope with health-related social factors.

The role involves contributing to a daily Plan of Care, guiding the care team, and using community resources. Requires MSW, valid driver’s license, and SW licensure in the state.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License, vehicle insurance, and access to dependable transportation
• Licensed Social Worker in the state of residence

Responsibilities

• Assists the patient, family, physician and health care team to understand personal, emotional and social factors relevant to health status
• Contributes to development of a comprehensive Plan of Care for patients on a daily basis
• Instructs team members on community resources to assist patients as needed
