Qualifications

• Bachelor's degree in social work accredited by the CSWE.
• Master's degree in social work preferred.
• Minimum of one year of experience in health care; hospice experience preferred.
• Licensed driver with insured vehicle as per state requirements.

Responsibilities

• Assess psychosocial status of patients and families/caregivers and communicate findings to the interdisciplinary team.
• Conduct social evaluations including family dynamics and caregiver abilities; plan intervention and provide crisis support as needed.
• Educate interdisciplinary group on cultural needs and family roles; respect traditions and taboos.
• Maintain clinical records and progress notes for all patients referred to social work.
• Assist in preparation of advance directives and provide information/referrals to patients and families.
• Oversee bereavement and volunteer programs within the agency and coordinate with community services.
