Job summary

AdvisaCare Home Health & Hospice is seeking a compassionate Medical Social Worker (MSW) to support patients and families facing life-limiting illness. This role offers flexible part-time/per diem opportunities in northern Michigan communities, with emphasis on dignity, comfort, and patient-centered care.

You will assess psychosocial needs, collaborate with the care team, and provide bereavement and counseling support to families as part of a dedicated hospice program.

Qualifications

• MSW degree from CSWE-accredited program.
• At least 1 year in Home Health Care (preferred).
• Hospice experience preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform initial psychosocial, emotional, spiritual and bereavement assessments and help develop care plans.
• Provide care under physician direction and participate in IDT conferences.
• Document visits, calls and referrals accurately in patient records.
• Conduct reassessments and provide counseling as required.
• Provide crisis intervention and family counseling when indicated.
• Advocate for patient/family within the care team.
• Develop and maintain community referrals to promote interagency cooperation.
• Offer ongoing counseling related to death and dying as needed.
• Participate in Bereavement Team as assigned.
