Job summary

Luminos Hospice is seeking a Hospice Social Worker to provide psychosocial assessments and counseling to patients and families, under physician direction, collaborating with the IDG/T to coordinate care and ensure holistic, compassionate support tailored to individual needs.

Qualified candidates hold an MSW, state licensure (LCSW/LMSW), and a minimum of two years in a healthcare setting, preferably hospice. Strong communication and resource coordination are essential for success in this role.

Qualifications

• Master’s degree in social work (MSW) from an accredited school of social work.
• Current state licensure as a social worker (LCSW, LMSW, or equivalent) required.
• Minimum two (2) years of experience in a healthcare setting, preferably in hospice or palliative care.
• Strong working knowledge of community resources and support services.

Responsibilities

• Conduct psychosocial assessments to evaluate emotional, social, and environmental factors impacting the patient and family.
• Provide emotional support and counseling to patients and families coping with end-of-life issues.
• Coordinate care with the Interdisciplinary Group/Team (IDG/T) to ensure holistic patient care.
• Assist families in understanding and managing grief and loss.
• Help families access community resources and services.
• Maintain documentation of social work services in accordance with hospice policies and regulatory requirements.
