Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families, provide interventions, and collaborate with an interdisciplinary team. Hospice experience of 1+ year is required, with a sign-on bonus opportunity.

The role focuses on resource support, short-term counseling, and crisis intervention within home-based hospice services. Licensure as LSW/LCSW and a valid driver's license are required.

Qualifications

• Master's degree in social work, plus licensure to practice.
• Hospice experience of 1+ years.
• Ability to work as part of a collaborative team.

Responsibilities

• Provide psychosocial assessments of patients and families to identify needs.
• Develop bereavement plan of care for families and caregivers.
• Communicate patient/family beliefs and values to the care team.
• Educate patients and families to overcome barriers and meet needs.
• Document assessments and interventions in the EMR.
• Implement and adjust social work plan in coordination with the IDT.
