Job summary

Carle Health is seeking a Home Services Social Worker to assess the psychosocial needs of patients and families and coordinate resources, short-term counseling, and crisis intervention as part of an interdisciplinary team.

The role requires hospice experience of 1+ years, and offers a sign-on bonus. You will conduct assessments, develop bereavement plans, educate families, document care, and collaborate on social work plans across the care continuum.

Qualifications

• Master's degree in Social Work is required.
• Active licensure as LSW or LCSW per IDFPR.
• Hospice experience of 1+ years is required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plans to address family needs.
• Educate patients and families, respecting faith and culture traditions.
• Complete visit notes and orders in a timely manner per policy.
• Document assessments and MSW interventions in the electronic medical record.
• Collaborate with IDT to develop and modify social work plans of care.
• Assist with transportation barriers and financial referrals as needed.
• Identify indicators of imminent death and address needs at time of death.
