Job summary

Carle Health is seeking a Home Services Social Worker to assess and address the psychosocial needs of patients and families. The role collaborates with multidisciplinary teams and may require varied hours based on census and program needs.

Hospice experience of 1+ year is required, with a sign-on bonus offered. The MSW route and licensure (LSW/LCSW) are essential, along with a valid driver’s license and auto insurance.

Qualifications

• MSW degree and current licensure in good standing.
• Prescribed hospice experience (1+ year) is required.
• Valid driver’s license and auto insurance as per agency policy.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plans to address needs of relatives and caregivers.
• Educate patients/families on learning needs and cultural beliefs.
• Document assessments and MSW interventions in the EMR.
• Coordinate with interdisciplinary team to plan care.
• Assist with advance directives and end-of-life decisions.
• Support transportation, food, and utility referrals as needed.
