Job summary

Qualifications

• Master's degree required in Social Work or related field.
• Licensed Social Worker (LSW) and Licensed Clinical Social Worker (LCSW) licensure in Illinois (IDFPR).
• Hospice experience preferred or required (1+ year).

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plans of care addressing family member needs.
• Educate patients and families, overcoming barriers and matching learning styles.
• Document assessments and interventions in the EMR per policy and timeframes.
• Collaborate with IDT to develop social work plans of care.
